Take ESR — erythrocyte sedimentation rate — one of the oldest and most reliable markers of inflammation. The kind your doctor might casually scan on a lab report and say, “You're fine, it’s under 20.”

 

What they don’t say is that the threshold — 20, 30 mm/hr or more — wasn’t chosen for optimization. It was chosen for pathology.
